Caring for people living with dementia requires compassion, patience, and tailored support to help maintain their quality of life. Families and carers receive a well-deserved break while their older person engages in activities that promote cognitive function, emotional well-being, and social connection. The Forget-me-not Dementia Care Programs are staffed by qualified Registered Nurses who specialise in dementia care.
AgeWell Dementia Care Programs prioritise person-centred care, focusing on individual needs, strengths, and preferences rather than just a diagnosis. Dementia Nurses promote independence through meaningful activities while creating a safe, supportive, and stimulating environment. Programs also offer caregiver support and resources, recognising their challenges. By focusing on abilities rather than limitations, these programs foster dignity, engagement, and an improved quality of life.
Qualified AgeWell staff assess behaviours and develop person-centred Behaviour Support Plans (BSPs) to help older people and NDIS consumers manage distress, unmet needs, or health-related behaviours. Created in consultation with the individual and their care partners, BSPs outline strategies to manage triggers, promote well-being, and ensure safety through tailored interventions, from pain management to engagement and environmental adjustments. By using proactive, evidence-based approaches, BSPs enhance quality of life while supporting carers in providing compassionate, effective care.
